A new enrollment form has to be completed for each member beginning September 1, 2012.  Please fill out the following form, print and mail (or deliver) to the Owen County Extension Office at 265 Ellis Hwy., Owenton, Kentucky 40359.  Or scan and submit via e-mail to staci.newsom@uky.edu
